Job summary

Active Care Group is seeking an experienced Care Manager based in Exeter for a hybrid role with home visits and client meetings. You will oversee complex care packages across the South West, travel up to two hours for client visits, and lead support workers within each package.

You will coordinate care plans, write assessments, and ensure high-quality, person-centered care. A driving licence and NVQ4+ in health and social care are required, with previous management experience advantageous.

Qualifications

• Excellent verbal and written communication skills.
• A passion for delivering high-quality client-focused care.
• Full UK Driving Licence.
• Experience in care at Team Leader level or above.
• Previous management experience would be an advantage.
• Willingness to travel across the South West region.

Responsibilities

• Take ownership of care packages in your areas.
• Collaborate with the Business Development Manager to review referrals.
• Produce assessment reports for client care packages.
• Ensure records are accurate and completed promptly.
• Build strong relationships with clients, families and external bodies.
• Deliver client-focused high-quality care at all times.
