Job summary

Active Care Group’s Active Assistance division is seeking an experienced Care Manager in Exeter for a Monday–Friday hybrid role combining home working with client visits across the South West. You will oversee complex care packages, support the team of Support Workers, and travel across Bristol, Cornwall and surrounding areas.

NVQ4+ in Health and Social Care, full UK driving license, and strong communication are essential.

Qualifications

• NVQ 4 or above in Health and Social Care is required.
• Full UK Driving License is required.
• Experience in care at Team Leader level or above.
• Previous management experience would be an advantage.
• Excellent verbal and written communication skills.

Responsibilities

• Have ownership of care packages in your areas.
• Review suitable referrals with the Business Development Manager.
• Produce suitable assessment reports for care packages for clients.
• Ensure all records are accurate and completed in a timely manner.
• Build excellent working relationships with clients, families and external bodies.
• Ensure we are providing client-focused high-quality care at all times.
