Job summary

Active Care Group in Thame is seeking a Senior Case Manager to lead complex caseloads, assess needs with clients and families, and coordinate rehabilitative services.

You will work with the Clinical Lead to promote best practice, develop case management plans, and maintain compliant documentation across the service.

Hybrid working with head office in Thame and travel to London/Home Counties as required; contribute to service growth and stakeholder engagement.

Qualifications

• Experience delivering case management for complex health conditions.
• Ability to assess clients and set goals with families.
• Must build relationships with referrers and partners.

Responsibilities

• Manage own caseload of clients with complex disabilities and conduct assessments.
• Develop and maintain case management plans with costed recommendations.
• Identify and coordinate rehabilitation services for clients.
• Ensure documentation is accurate and compliant with regulations.
