Job summary

Active Care Group is seeking an experienced Care Manager based in Exeter for a hybrid role with home visits and client meetings. You will oversee complex care packages across the South West, travel up to two hours for client visits, and lead support workers within each package.

You will coordinate care plans, write assessments, and ensure high-quality, person-centered care. A driving licence and NVQ4+ in health and social care are required, with previous management experience advantageous.

This exciting and varied position requires a willingness to travel across the region, including Bristol, Cornwall, and surrounding areas, with journeys of up to two hours when required.You will work closely with clients who have a wide range of complex needs, ensuring they receive safe, high-quality, person-centred care that promotes independence, dignity, and positive outcomes. Alongside supporting our clients, you will provide guidance, leadership, and ongoing support to Support Workers within each package, helping them develop their skills and deliver exceptional care.This is a role that can be demanding at times, requiring resilience, flexibility, and excellent problem-solving skills.
