Job summary

Active Care Group is seeking a Clinical Manager to take clinical ownership of care packages across the region, collaborating with Care Managers, Co-ordinators, and multidisciplinary teams to ensure high-quality, personalised care in the home.

You will lead clinical governance, assess new complex care packages, and participate in MDTs while driving exceptional standards and patient outcomes in a growing home-based care business.

Qualifications

• Registered Nurse (RN) with current NMC registration.
• Advanced qualifications in clinical governance, quality assurance, or risk management.
• Experience in a clinical leadership role within complex/domiciliary/community care.
• Proven track record of driving clinical excellence and regulatory outcomes (e.g., CQC).

Responsibilities

• Clinical management of caseload with clear care plans and contingency planning.
• Maintain skills and training; respond to operational needs including occasional out-of-hours support.
• New client assessments and mobilisation of complex care packages.
• Attend MDT meetings with internal/external stakeholders to support clinical care.
• Initiate new ideas and provide flexible, responsive leadership.
• Liaise with funders and attend case conferences/MDT meetings as required.
• Support investigations and disciplinary processes with Care Manager and Registered Manager.
