Job summary

Think FE Ltd is recruiting a Transitions Assistant for a Northamptonshire college, supporting students and families to ensure a smooth transition into college and personalised plans. You will work with Local Authorities, schools and college staff, with a focus on SEND advocacy and ensuring EHCPs are reflected in funding and arrangements.

Qualifications

• Experience working with SEND students or young people.
• Good understanding of EHCPs.
• Strong communication and relationship-building skills.
• Supportive, practical and organised approach.
• Full UK driving licence and access to own vehicle.

Responsibilities

• Support students from initial enquiry through to settling on the right course.
• Develop tailored transition plans based on individual needs.
• Liaise with parents, carers, Local Authorities, schools and college teams.
• Review EHCP information and funding arrangements.
• Advocate for SEND learners and ensure appropriate support is in place.
• Visit local schools and other settings as required.
