Job summary

Exeter College is seeking a Transitions Link Officer to act as the vital link between schools, families and the College. You will ensure learners with EHCPs and additional learning support needs experience a smooth transition into further education.

You'll gather and share high-quality information, work with EHCP and Learning Support teams, and support panel decisions to help learners thrive. This role emphasizes inclusion and proactive collaboration with families and professionals.

Qualifications

• Has experience working with young people with SEND, EHCPs or additional learning support needs.
• Understands the transition process from school into further education.
• Builds positive relationships quickly and communicates confidently with a wide range of people.
• Is highly organised, with excellent attention to detail and the ability to manage sensitive information accurately.

Responsibilities

• Building strong relationships with schools, SENCOs, specialist settings, families and external agencies.
• Gathering, recording and sharing accurate information about prospective learners with EHCPs or additional learning support needs.
• Supporting the College's transition planning by ensuring Learning Support teams have the information they need to prepare for learner success.
• Contributing to consult responses and supporting decision-making through the College's agreed panel process.
• Providing advice and guidance to families and professionals about transition pathways, learning support and the College's processes.
• Acting as a positive ambassador for the College, promoting an inclusive and learner-centred approach.
