Job summary

New City College is seeking a Transitions Coordinator to support students with SEND, EHCPs, and high-needs learners as they move from school into further education. You will be the main contact for students, families, schools, local authorities and agencies, delivering person-centred guidance and building strong partnerships across campuses.

Qualifications

  • Experience developing positive relationships with a wide range of stakeholders, including schools, parents/carers and external agencies.
  • Knowledge of SEND provision, learner support and progression pathways within education.
  • Strong communication, presentation and organisational skills.
  • Ability to work collaboratively, manage competing priorities and maintain accurate records.
  • Commitment to safeguarding, equality, diversity and inclusion.
  • Willingness to travel between campuses and local schools as required.

Responsibilities

  • Provide person-centred information, advice and guidance on learning pathways and support services.
  • Coordinate transition activities including school visits, learner observations, annual reviews, interviews and enrolment support.
  • Work with curriculum and support teams to ensure learners receive appropriate support and resources.
  • Monitor EHCP related processes to ensure compliance and successful outcomes.
  • Represent the college at open days and events, promoting inclusive education.
