Job summary

Abingdon and Witney College in Witney is seeking a proactive Transition Coordinator to support learners with EHCPs as they move into, through, and beyond the College. You will work as a key link between schools, families, Local Authorities, and the SEND team to ensure each learner receives guidance, support, and opportunities to succeed.

The role requires experience with SEND, knowledge of legislation, and strong communication and organisational skills.

Qualifications

Experience working with young people with SEND.
Knowledge of SEND legislation and the SEND Code of Practice.
Strong communication and organisational skills.
Experience working collaboratively with professionals and stakeholders.
Committed to inclusion, safeguarding, and student success.

Responsibilities

Coordinate personalised transition plans for learners with SEND.
Build strong relationships with schools, families, Local Authorities, and external agencies.
Attend reviews, transition meetings, and stakeholder events.
Support learners in preparing for adulthood, further learning, employment, apprenticeships, or independent living.
Organise transition events, campus visits, and SEND-focused activities.
