Job summary

Think FE Ltd in Northamptonshire is seeking a Transitions Assistant to support students with SEND as they move into college. You will work closely with families, Local Authorities and schools to ensure a smooth, personalised transition and advocate for EHCP funding and support.

You will develop tailored transition plans, liaise with teachers and external organisations, and help learners feel confident and prepared for further education. Previous SEND experience is essential.

Qualifications

  • Experience working with SEND students or young people.
  • Good understanding of EHCPs.
  • Strong communication and relationship-building skills.
  • Supportive, practical and organised approach.
  • Full UK driving licence and access to your own vehicle.

Transitions Assistant

Location: Northamptonshire

Salary: £26,366 - £27,168 (£21,802.65 - £22,465.84 per annum pro rata)

Contract: Fixed term until July 2027

Interviews: Week commencing 26 October 2026

We are recruiting for a Transitions Assistant to join a supportive further education college in Northamptonshire.

You will work closely with students, families, Local Authorities, schools and college staff to ensure prospective learners receive a smooth, personalised transition into college.

A key focus of the role will be supporting and advocating for students with SEND, ensuring transition plans reflect individual needs, EHCPs and funding arrangements.

The role

You will:

  • Support students from initial enquiry through to becoming settled on the right course
  • Develop tailored transition plans based on individual needs
  • Liaise with parents, carers, Local Authorities, schools and college teams
  • Review and work with EHCP information
  • Advocate for SEND learners and ensure appropriate support is in place
