Job summary

New City College is seeking a Transitions Coordinator to support students with SEND, EHCPs and other high-needs learners as they move from school into further education. You will act as the main point of contact for students, families, schools, local authorities, and external agencies, delivering person-centred guidance to ensure smooth, inclusive transitions.

• Experience supporting SEND/EHCP learners and familiarity with safeguarding in education.
• Ability to build partnerships with schools, families, local authorities and external agencies.
• Strong communication, organisation and record-keeping skills.

Responsibilities

• Act as the primary contact for high-needs learners, parents/carers, schools, local authorities and external agencies throughout the transition process.
• Develop and maintain partnerships with schools and local authorities to support transition planning.
• Provide information, advice and guidance on suitable learning pathways and support services.
• Coordinate transition activities including school visits, learner observations, annual reviews, interviews and enrolment support.
• Work with curriculum and support teams to ensure appropriate support and resources for learners.
• Monitor learner information, support requirements and EHCP-related processes to ensure compliance and successful outcomes.
• Represent the college at open days, school events and promotional activities, promoting inclusive education and progression opportunities.
