Job summary

New City College is seeking a Transitions Coordinator to support students with SEND, EHCPs and other high-needs learners moving from school into further education. You will be the main point of contact for students, families, schools and local authorities, delivering person-centred advice and building strong partnerships.

You will work with curriculum and support teams to ensure appropriate adjustments, coordinate visits and enrolment, and monitor EHCP processes to meet statutory requirements.

Qualifications

• Experience supporting SEND learners and EHCP processes.
• Ability to build partnerships with schools, local authorities and external agencies.
• Provide impartial information, advice and guidance on learning pathways.
• Coordinate transition activities including visits, reviews and enrolment support.
• Work with curriculum and support teams to ensure appropriate adjustments.
• Commitment to safeguarding, equality, diversity and inclusion.

Responsibilities

• Support students with SEND through the transition to college.
• Coordinate school visits, annual reviews and enrolment support.
• Provide person-centred information, advice and guidance.
• Represent the college at events to promote inclusive education.
