Job summary
Northumberland College is recruiting a Transition Advisor for SEND to develop and coordinate early transition planning for students with SEND. You will work closely with schools and partner agencies to design targeted support and prepare learners for college life, ensuring EHCP processes are followed and progression pathways are clear.

Qualifications
- Level 2 or 3 qualification in learning support or equivalent.
- Level 2 literacy and numeracy.
- Experience working with schools and external agencies to support transitions.
Responsibilities
- Work in partnership with schools to plan and deliver early transition support
- Assess student needs and develop tailored transition plans
- Manage a caseload of SEN students and track progress
- Contribute to EHCP processes, meetings, and reviews
- Liaise with families, staff, and external agencies
- Maintain accurate records and update relevant databases
