A local government authority in Leicester is seeking a Case Manager to draft and amend Education Health and Care Plans (EHCPs) and manage a high caseload. The role emphasizes effective communication with parents, schools, and various stakeholders to ensure positive outcomes for children. Qualifications include NVQ Level 4 in a relevant subject. This position requires flexible working arrangements and a commitment to quality service delivery while adhering to relevant legislative requirements.
We are advertising for Case Manager positions available within our busy SENA Service. The posts will be based in our assessment review and Placement teams and you may be required to work flexibly across other teams as demand requires at an officer level. About the Role The post holder will be drafting / amending Education Health and Care Plans (EHCPs) following the advice / review meeting. You will have a large caseload and you will be working within Code of Practice timescales. You will also carry out a full range of Case Manager duties including managing a high level parental contact finding solutions to problems covering a duty system on a rota and acting as a point of contact for internal and external contacts. To use professional expertise to provide advice and support to a range of colleagues within Children and Families Services and partner agencies to enhance positive outcomes for children and young people using performance data and intelligence within a multi-agency approach. Liaising with a range of external stakeholders including schools experts for example Educational Psychologists Specialist Teachers to ensure that any and all relevant information is available for the EHCP review process. Effective case management often requiring multi-disciplinary working with external services such as Social Care Health Services other local authorities and specialist educational settings to deliver the best outcomes for children and young people. Effective problem solving information gathering and officer level decision making often under short time constraints. Working directly with parents children and young people schools and linked agencies within the statutory and legal framework related to the Special Educational Needs EHCPs. To model good case work practice and to demonstrate strong team worker skills via coaching joint working and contributing to the development and delivery of training. Ensuring service performance quality standards are adhered to within the framework for Leicestershire County Councils priorities for children and families; any relevant Government legislative requirements and objective setting based on local commissioning strategic imperatives and performance frameworks. Maintaining effective communication and building strong professional relationships with parents schools central services health and social care professionals regarding individual cases. To lead and support attendance and information sharing in a range of formal meetings including person-centred co-production meetings as agreed with the Team Manager. A DBS enhanced check for a regulated activity is required for this post.
