Bridgewater Community NHS Foundation Trust have an exciting opportunity for a caring and ambitious SCPHN school nurse to join our school nursing team and be an integral part of our 3 well established integrated teams within our Warrington 0-19 service. Main duties of the job

Be a qualified Specialist Community Public Health Nurse (School Nursing).

Provide supportive leadership within the 0-19 team.

Be an autonomous practitioner who can support the needs of the team and service.

Take part in service development, promote innovation and demonstrate skills in public health.

Have a flexible and conscientious approach to tasks.

Job responsibilities
  1. The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
  2. Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies.
  3. Lead on the recommended health assessment and reviews including: Reception/Year 1 (age 4-5) school entry assessment (transition from early years health care plan to school entry questionnaire); providing Year 6/7 (age 10-12) assessment at transition from primary to secondary school.
  4. Supporting mid-teens reviews, when young people are embarking on the next transition stage as required in national and local specifications.
  5. Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met.
  6. Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age.
  7. Contribute to the delivery of the national immunisation programme for school aged children.
  8. Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs.
  9. Perform home visits to children and families advising on all aspects of health as appropriate.
  10. Work in partnership with other key stakeholders to safeguard and protect children and young people.
  11. Contribute and take part in regular team meetings and multi-agency groups as appropriate.
  12. Work collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family.
  13. Provide training where necessary to young people, parents, teaching personnel and other professionals.
  14. Provide signposting to other services and agencies, referring and liaising appropriately.
  15. Take and act upon referrals from other agencies and services, as appropriate.
  16. Provide a confidential support and advice service that is easily accessible for young people within a high school or other venue on a regular basis.
  17. Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, working in partnership with school staff and with other statutory and voluntary agencies.
  18. Utilise opportunities to promote healthy lifestyles, positive mental health and the public health agenda.
  19. Ensuring children not in employment, education or training, or children educated at home receive the universal offer.
  20. Complete PDRs and return to work pro-forma as required to the designated named staff within the team.
  21. Ensure performance data is collated in a timely manner.

Clinical

  • Maintain active status on NMC register acting in accordance with the professional code and guiding documents in adherence with Revalidation Framework.
  • Develop a working knowledge and experience of IT systems.
  • Maintain contemporaneous, factual records in accordance with Trust guidelines and policies.
  • Be aware of current working practices in accordance with Trust policies and procedures.
  • Manage their time effectively to ensure prioritisation of workload to achieve agreed deadlines.
  • Be responsible for own professional updating and keep abreast of current research-based practice.
  • Participate in study days as appropriate and be aware of current developments and research within the area of practice.
  • Observing the complaints and disciplinary procedures in accordance with the Trust policy.
  • Report sickness absence and return relevant forms to line manager.
  • Maintain portfolio of professional development.

Leadership

  • Provide supervision, where appropriate, to support the skill mix team.
  • Undertake appropriate delegation within team.
  • Support other School Nurses and staff within the team, including the induction of new staff members. Ensure effective learning experience and opportunities to achieve learning outcomes for the skill mix team.
  • Develop specialist knowledge and expertise in an area of practice appropriate to service needs and act as a resource to colleagues and other professionals in the chosen area (e.g., Asthma, Behaviour Management, Allergy).
  • Develop and implement policies, procedures and guidelines relevant to own area of work. Provide advice where services are developing.

Research and Audit

  • Identify and undertake clinical audit, evaluation and dissemination of results through presentation to professional groups and publications, to improve effectiveness of care and ensure effective and efficient use of resources.

Supervision and Development

  • Identify and communicate training needs through the Personal Development Review (PDR) process.
  • Provide mentorship and preceptorship to students and newly qualified staff as required.
  • Undertake mandatory and statutory training and updating as required by the Trust.

Communication

  • Communicate effectively with individuals, families and professionals in planning and delivery of care, demonstrating empathy and reassurance in difficult and highly sensitive situations.
  • Maintain contemporaneous and accurate Bridgewater Community NHS Trust records, observing the Data Protection Act and maintaining confidentiality in accordance with NMC and Trust guidelines.
