Job summary

Hampshire County Council is recruiting a Reablement Service Practitioner to work across disciplines, supporting people to live independently at home wherever possible. You will triage referrals, assess needs, plan interventions and work with a multi-disciplinary team to promote safe, dignified care.

The role emphasizes clear communication, empathy, and safeguarding responsibilities, with a mix of day/evening shifts on a rolling rota and a hybrid working pattern.

Qualifications

• Maths and English at GCSE or Functional Skills (Level 2) is required.
• Experience in Reablement, Rehabilitation, Health or Social Care setting is preferred.
• General knowledge of the Care Act 2014 and Mental Capacity Act 2005 is preferred.

Responsibilities

• Triaging referrals, completing holistic assessments of need and planning interventions.
• Using a strengths-based approach to provide advice and information throughout the reablement journey.
• Building positive relationships with service users and networks; safeguarding support.
