Job summary

Hampshire County Council is seeking a Community Reablement Assistant (Level 2) to support adults over 18 in Hampshire to maximise independence and stay at home. The role covers a Central Hampshire area including Winchester, with hybrid working arrangements and potential travel to nearby communities.

You will conduct home visits, perform reablement interventions, assess risks, set goals and monitor progress while coaching colleagues and coordinating with health and social care teams.

Qualifications

• Experience in a social or health care setting.
• Strong interpersonal and communication skills.
• Willingness to travel within Central Hampshire area.

Responsibilities

• Conduct initial home visits and independently perform reablement interventions to restore independence.
• Perform risk assessments, set goals, update reablement plans for Level 1 Assistants and monitor service user progress.
• Coach and mentor Community Reablement Assistants and assist teams with activities.
