Hampshire County Council is seeking a Reablement Service Practitioner to work across disciplines within the Reablement service, supporting people to live independently at home wherever possible and in the least restrictive way.

You’ll work with a multi-disciplinary team to assess needs, plan interventions, and provide information to enable informed decisions, while building strong relationships with service users and their networks.

Qualifications

• Experience in Reablement/Health or Social Care setting preferred.
• Knowledge of Care Act 2014 and Mental Capacity Act 2005 preferred.
• Excellent communication and teamwork skills.
• Detail-oriented with ability to assess risks and analyse information.
• Demonstrates patience, compassion and empathy in interactions with service users.

Responsabilités

• Triage referrals, complete holistic assessments of need and plan supportive interventions.
• Provide advice and information to enable problem solving during the reablement journey.
• Build positive relationships with service users, carers and local healthcare professionals.
• Support safeguarding cases alongside professional practitioners.
