Job summary

Hampshire County Council is seeking a Community Reablement Assistant (Level 2) to support adults over 18 in Hampshire, helping them regain independence and remain at home. You’ll conduct initial home visits, carry out reablement interventions and work with a multi-disciplinary team including Occupational Therapists and Health colleagues.

You should have social or health care experience, a patient, empathetic approach, and IT skills (Microsoft 365).

Qualifications

• Experience in social or health care setting.
• Understanding reablement needs and aims.
• Patient, empathetic and respectful approach.
• Good interpersonal and communication skills.

Responsibilities

• Provide person-centred care during initial home visits and reablement interventions.
• Carry out risk assessments and update reablement plans.
• Coach and mentor Community Reablement Assistants and support team leads.
