Job summary

West Berkshire Council seeks a dedicated Reablement Practitioner within the Adult Social Care Service to help residents regain independence through tailored support and careful reassessment. You will assess needs, develop reablement plans, monitor progress, and work with carers and professionals to remove barriers to independence, promoting self-care and empowered living in the community.
