Job summary

Qualifications

• Experience in social or health care settings.
• Understanding of reablement programmes and outcomes.
• Ability to provide person-centred care with dignity and respect.
• Proficient IT skills including Word, Outlook and Excel.

Responsibilities

• Carry out initial home visits and implement reablement interventions to promote independence.
• Conduct risk assessments and set goals within reablement plans.
• Coach and mentor junior team members and support team leaders and therapists.
