Job summary

Hampshire County Council invites applications for a Community Reablement Assistant (Level 2) to join the Reablement Team and support adults over 18 in Hampshire who need extra care. • Experience in care or support roles preferred.
• Ability to perform home visits and support reablement activities.
• Excellent communication and empathetic approach with adults.

Responsibilities

• Provide person-centred care including initial home visits and reablement interventions to promote independence.
• Deliver outcomes by conducting risk assessments, setting goals, and updating plans for assistants; monitor progress.
• Support operational delivery by coaching and mentoring team members and assisting leaders and therapists.
