Job summary

AMS Case Management in Bolton, Greater Manchester, seeks a qualified Case Manager to assess, plan and monitor rehabilitation needs for clients across the North West and North East, working with a multidisciplinary team to set SMART goals.

The role is full-time with hybrid/remote working and a salary of £40,000–£50,000 per year, plus a company pension, mileage and client expenses, with UK travel as required.

Qualifications

• Current registration with a relevant professional body.
• Minimum four years post-qualification experience, preferably with brain injury, spinal injury, physical disabilities or orthopaedics.
• Full clean driving licence and access to a car.
• Willingness to travel across the UK as required for individual cases.
• Acceptable standard of home office.
• Clean DBS check at enhanced level.
• Experience working within community, hospital, or private rehabilitation settings.

Responsibilities

• Assess the client’s needs and develop appropriate individual case management plans.
• Produce and implement proposals and recommendations for intervention including costs to instructing parties.
• Conduct risk assessments and, where appropriate, produce a risk management plan.
• Develop and prioritise client SMART goals and timetabled actions.
• Research and engage relevant specialists/therapists and monitor progress.
• Support client engagement in day-to-day activities to meet rehabilitation goals.
• Liaise with concerned parties, deliver brain injury training where appropriate, and review progress against goals.
• Monitor costs against proposed budgets to ensure alignment with allocated resources.
• Provide and submit timely progress summaries and case notes for billing.
