AMS Case Management is seeking an Associate Rehabilitation Case Manager to join our Bolton-based team. This self-employed role covers the North West and North East of England, with offices in Bolton, offering flexibility in caseload and revenue potential.
You will assess needs, develop rehab plans, coordinate multi-disciplinary input, and ensure timely progress while managing costs and outcomes for clients. A strong clinical background and UK travel readiness are essential.

This is an exciting opportunity for a dynamic social worker / occupational therapist / physiotherapist / nurse with previous case management experience to join our case management team on an associate basis.
All successful candidates will be required to provide evidence of their right to work within the UK.
Due to the nature of this work, successful candidates will be subject to an enhanced Disclosure and Barring Service check.
Appointment to the post is subject to satisfactory references, registration checks, proof of holding a full UK driving licence and have been driving for at least 12 months.
