Job summary

Brownbill Case Management, part of the Active Care Group, is recruiting a Clinical Case Manager to assess needs, plan care, and coordinate multidisciplinary teams in a hybrid setting. You will develop tailored rehabilitation plans, work with clients and their families, and supervise support workers to deliver high-quality care in the community and home environments.

The role requires a UK health qualification, experience with neurological conditions, and strong communication skills.

Qualifications

• Hold a UK professional Health qualification (nurse, OT, physio, social worker, etc.).
• Experience with neurological conditions and community-based care.
• Knowledge of MCA/DoLS and safeguarding frameworks.
• Strong communication and IT skills.

Responsibilities

• Provide and maintain a case management service for clients and families.
• Assess needs and risks; produce case management plans with client consent.
• Develop action plans with timelines tailored to each client.
• Coordinate and supervise care/support workers; recruit and manage staff as needed.
• Ensure service user needs and business objectives are met.
• Work flexibly across multi-site environments.
