Active Care Group is recruiting a Case Manager for the Corbridge area in North East England. The role is hybrid, with office time and client visits across a wide geographic area. A full UK driving licence is required and travel is a regular part of the job.

A DBS check is required, and the role involves leading multi-disciplinary teams to assess rehabilitation needs and coordinate services. Candidates should be professionally qualified in Social Work, Nursing or a related Allied Health field,

Qualifications

• Must be professionally qualified as one of the listed professions.
• Minimum of 3 years’ experience in the field.
• Previous case management experience.
• Excellent report writing skills.
• Full UK driving license.
• Successful candidates will be required to undergo an Enhanced DBS.

Responsibilities

• Assess rehabilitation needs and coordinate services for cases.
• Lead multi-disciplinary teams to deliver community-based care.
• Advocate for clients and navigate health and social care systems.
• Monitor progress and adapt plans as needs evolve.
