Job summary

Active Care Group in Wolverhampton is seeking a Senior Case Manager to oversee a caseload of clients with complex health needs, working with families to set goals and arrange services. You will partner with the Clinical Lead to ensure high-quality care and support senior leadership across the service.

The role is hybrid, with coverage across London, Home Counties, Essex, Kent, Midlands and the South, offering a pathway to develop practice under experienced leadership and ongoing professional

Qualifications

• Experience delivering case management for complex health conditions.
• Ability to develop care plans and coordinate services.
• Knowledge of regulatory standards and quality processes.

Responsibilities

• Manage your own caseload with complex disabilities and set goals with clients and families.
• Develop case management plans with costed recommendations.
• Identify and commission rehabilitation services and monitor delivery.
• Maintain accurate client documentation and regulatory compliance.
• Lead stakeholder relationships and provide clinical support to colleagues.
