Job summary

Active Care Group is seeking an experienced Case Manager to cover a caseload across Norfolk and Suffolk. The role involves detailed assessments, developing individual case management plans and coordinating rehabilitation services with multidisciplinary teams.

Qualifications

• Experience as Case Manager handling complex clients.
• Strong assessment, report-writing and care planning skills.
• Experience coordinating multidisciplinary teams and commissioning services.

Responsibilities

• Undertake detailed assessments and identify rehabilitation and support needs.
• Develop and regularly review individualized case management plans.
• Commission and coordinate rehabilitation services across sectors.
• Coordinate multidisciplinary teams and monitor interventions.
• Undertake risk assessments and develop care packages as required.
• Liaise with clients, families, solicitors, Deputies and other professionals.
• Produce assessment and review reports and maintain records.
• Support clients with housing, equipment, education, leisure and community needs.
