Job summary

AJ Case Management is seeking a Case Manager to support clients with complex disabilities, including brain injury, at home, work, and in education. You will undertake needs assessments, set intervention goals with clients and families, and provide updates to care teams.

The role offers remote and flexible working within a UK travel radius, with either full-time or part-time arrangements (minimum 3 days/week) and pay dependent on experience.

Qualifications

• Registration with the appropriate professional body (e.g. SWE/HCPC/NMC).
• Experience with brain injury, neurological and other complex conditions.
• Excellent verbal and written communication, organisational and problem‑solving skills.

Responsibilities

• Provide Case Management services to clients (children and/or adults) with complex disabilities.
• Undertake needs assessments and set goals for intervention with clients and families where appropriate.
• Provide regular updated reports to relevant parties in AJ Case Management standards.
