Herefordshire and Worcestershire Health and Care NHS Trust is recruiting a Community Support Worker to join its multidisciplinary core team. The post centres on supporting a caseload of patients through recovery-focused interventions, delivered alongside registered professionals, and signposting people to community services.

The role is 30 hours over four weekday shifts, with no weekends or bank holidays, and needs flexible working.

Qualifications

• Level 3 qualification in care, or equivalent qualification or experience.
• Level 2 qualification in care, or equivalent, with level 3 expected on appointment.
• Experience in one-to-one work and personal care tasks.

Responsabilités

• Work as a key member of the multidisciplinary core team.
• Manage a caseload of patients.
• Undertake recovery-focused interventions.
• Support registered professionals to deliver evidence-based interventions.
• Attend weekly MDT meetings and professionals' meetings.
• Refer and signpost patients to appropriate services based on need.
• Signpost to community-based services to reduce social isolation and improve mental wellbeing.
• Provide support and guidance to carers of patients.
