Job summary

Change Grow Live is recruiting a Recovery Coordinator for the Intensive Support Team in Nottinghamshire. You will manage a caseload of service users with complex needs, coordinating with multiple agencies to provide wrap-around support and develop risk-managed plans.

The role requires experience in substance misuse, safeguarding, and facilitating multi-agency meetings. You will work with partners and health services, maintaining records and contributing to service development.

Qualifications

• Knowledge of substance misuse, alcohol misuse, and mental health issues.
• Experience with multi-agency collaboration and safeguarding.
• Ability to work with complex service users and maintain boundaries.
• Experience facilitating multi-agency meetings.
• Experience working with statutory and voluntary agencies.
• Good administrative, recording and reporting skills.
• Excellent communication, negotiation and advocacy.
• Strong IT skills and Microsoft Office proficiency.
• Full driving license and access to a vehicle.

Responsibilities

• Manage a caseload of service users accepted via Intensive Support Panel.
• Build relationships and support access to services.
• Work with core housing and external partners.
• Manage service user plans and assess risk.
• Advise service users on substance/alcohol use using motivational interviewing.
• Coordinate with Mental Health Agencies to address mental health needs.
• Contribute to ongoing access and updates to service user records.
• Participate in shift patterns including evenings/weekends as needed.
