Change Grow Live in Norwich is recruiting a Recovery Coordinator to deliver recovery-focused interventions, support service users across their treatment journey and coordinate care to meet individual needs. The role requires travel and a full driving licence, with 37.5 hours per week and permanent, full-time employment.

You will work with service users, partner agencies and colleagues to assess needs, develop recovery plans and coordinate care, while maintaining accurate records and ensuring

Formación

• Strong understanding of substance misuse issues and experience in a similar field.
• Experience of working with groups/pods and managing group dynamics.
• Excellent verbal and written communication with good IT skills.
• Proactive, able to work on own initiative with strong time management.

Responsabilidades

• Support service users from entry through treatment and recovery journey.
• Screening, assessment, recovery planning and onward referral.
• Reduce drug and alcohol-related harm to service users and the wider community.
• Promote involvement of carers, service users, and community.
• Advocate for access to partnership services and support social reintegration.
