Zusammenfassung

Change Grow Live in Norwich is seeking a Recovery Coordinator to manage a caseload of individuals affected by drugs and alcohol. You will deliver recovery-focused interventions, undertake assessments, develop recovery plans and coordinate care across Norfolk localities.

The role requires a valid driving licence for regular travel, with 37.5 hours per week on a permanent full-time basis. Salary range is set within CGL points 23–28, with pro rata terms for part-time working needs.

Qualifikationen

• Experience in health, social care or a related field preferred.
• Ability to build positive relationships with service users and partners.
• Must be able to travel regularly and have a driving licence/vehicle.
• Strong recordings and IT literacy for case notes and reporting.

Aufgaben

• Support service users from entry into the service through their treatment/recovery journey.
• Provide screening, assessment and recovery planning with onward referral.
• Reduce drug and alcohol related harm for service users and community.
• Promote carer, service user and community involvement.
• Advocate for access to partner services.
• Work with service users to support social reintegration and peer support.
• Research, design and prepare materials for groups/pods.
• Deliver structured content within group settings at different recovery stages.
• Debrief with co-facilitators after sessions and review progress with service users.
• Tailor workshops to meet service user needs (e.g., anger management, sleep, relationships).
