Job summary

The Care Bureau Ltd. is seeking a Reablement Support Worker to help people transition from hospital to home across Warwickshire. You will support daily activities, personal care, meals and medication prompts, and assist clients to regain independence following discharge.

You will join a team focused on safety, independence and confidence-building, with paid induction and ongoing support. A driving licence and access to a car are required for travel between visits.

Qualifications

• Compassionate, patient and reliable with a genuine desire to make a difference to people’s lives.
• Training provided and support offered regardless of prior care experience.
• Willing to travel between visits across Warwickshire.
• You hold a full UK driving licence and have access to your own car with business insurance.
• Available to work shifts including early mornings, evenings and weekends.
• Willing to undergo an enhanced DBS check.

Responsibilities

• Welcoming people home from hospital and helping them settle in safely
• Supporting personal care, meals, medication prompts and daily routines
• Helping people rebuild confidence and practise everyday skills, in line with their reablement plan
• Spotting changes in health or wellbeing and reporting them promptly
• Keeping clear, accurate records and working with the wider team and health professionals
