Job summary

The Care Bureau Ltd is hiring a Reablement Support Worker to help people transition from hospital to home across Warwickshire. You will support with personal care, meals and daily routines, and help clients regain independence following hospital discharge.

Role involves flexible shifts, driving between visits, DBS checks, and full induction with paid training. A compassionate, reliable approach is essential to build confidence and dignity in clients’ own homes.

Reablement Support Worker (Hospital to Home)

Location: Warwickshire (flexible across the county)

Contract: Guaranteed hours, block shifts

Pay: £12.71 to £12.80 per hour

About the role

Leaving hospital can be daunting. As a Reablement Support Worker with Care Bureau, you'll be the friendly face who gets people safely home and settled again. Like domiciliary care, you'll work in people's own homes. The difference is that you'll support people straight after a hospital discharge. You'll help them settle back in, regain confidence and get back to doing things for themselves, so the aim is to build independence rather than create dependence.

What you'll be doing

• Welcoming people home from hospital and helping them settle in safely
• Supporting personal care, meals, medication prompts and daily routines
• Helping people rebuild their confidence and practise everyday skills, in line with their reablement plan
• Spotting any changes in someone's health or wellbeing and reporting them promptly
• Keeping clear, accurate records and working closely with the wider team and health professionals

• You’re available to work shifts, including early mornings, evenings and weekends.
• You’re willing to undergo an enhanced DBS check, which we’ll arrange.
