Job summary

The Care Bureau Ltd is recruiting a Reablement Support Worker to help people transfer from hospital to home and regain independence. You will conduct home visits across Warwickshire, supporting daily activities and medication prompts, while following a personalised reablement plan.

Training is provided and you will join a friendly team. Guaranteed hours contract with block shifts supports reliable earnings, with mileage paid between visits and ongoing opportunities for progression into senior or

Qualifications

• Compassionate, patient and reliable.
• Training and support provided, including moving and handling, medication.
• Full UK driving licence and access to own car with business insurance.
• Willing to undergo an enhanced DBS check.

Responsibilities

• Welcome people home from hospital and help them settle in safely.
• Support personal care, meals, medication prompts and daily routines.
• Help rebuild confidence and practise everyday skills per reablement plan.
• Spot health and wellbeing changes and report them promptly.
• Maintain clear records and liaise with wider team and health professionals.
