The Care Bureau Ltd is hiring a Reablement Support Worker to help people transition from hospital to home across Warwickshire. You will support daily routines, meals and personal care while promoting independence in a patient, home-based setting.

You will work flexible hours under a guaranteed hours contract with block shifts, receive paid mileage, full induction and ongoing support from an office team, and opportunities for progression. DBS checks and driving are required.

Qualifications

• Compassionate, patient and reliable, with a genuine desire to help people.
• Willing to travel between visits across Warwickshire and hold a full UK driving licence.
• Willing to undergo an enhanced DBS check and complete training.

Responsabilités

• Welcoming people home from hospital and helping them settle in safely.
• Supporting personal care, meals, medication prompts and daily routines.
• Helping people rebuild confidence and practise everyday skills per reablement plan.
• Spotting changes in health or wellbeing and reporting promptly.
• Keeping clear records and coordinating with the team and health professionals.

Reablement Support Worker (Hospital to Home)

Location: Warwickshire (flexible across the county)

Contract: Guaranteed hours, block shifts

Pay: £12.71 to £12.80 per hour

About the role

Leaving hospital can be daunting. As a Reablement Support Worker with Care Bureau, you'll be the friendly face who gets people safely home and settled again. Like domiciliary care, you'll work in people's own homes. The difference is that you'll support people straight after a hospital discharge. You'll help them settle back in, regain confidence and get back to doing things for themselves, so the aim is to build independence rather than create dependence.

What you'll be doing

• Welcoming people home from hospital and helping them settle in safely
• Supporting personal care, meals, medication prompts and daily routines
• Helping people rebuild their confidence and practise everyday skills, in line with their reablement plan
• Spotting any changes in someone's health or wellbeing and reporting them promptly
• Keeping clear, accurate records and working closely with the wider team and health professionals
