Zusammenfassung

The Care Bureau Ltd is seeking a Reablement Support Worker to help clients transition from hospital to home across Warwickshire. You will work in clients' homes, supporting independence and settling in after discharge.

You will assist with personal care, meals, medication prompts and daily routines, while monitoring wellbeing and reporting changes to the health team. Comprehensive induction and ongoing training are provided.

Location: Warwickshire (flexible across the county)

Contract: Guaranteed hours, block shifts

Pay: £12.71 to £12.80 per hour

About the role

Leaving hospital can be daunting. As a Reablement Support Worker with Care Bureau, you'll be the friendly face who gets people safely home and settled again. Like domiciliary care, you'll work in people's own homes. The difference is that you'll support people straight after a hospital discharge. You'll help them settle back in, regain confidence and get back to doing things for themselves, so the aim is to build independence rather than create dependence.

What you'll be doing

• Welcoming people home from hospital and helping them settle in safely
• Supporting personal care, meals, medication prompts and daily routines
• Helping people rebuild their confidence and practise everyday skills, in line with their reablement plan
• Spotting any changes in someone's health or wellbeing and reporting them promptly
• Keeping clear, accurate records and working closely with the wider team and health professionals
