Job summary

The Care Bureau Ltd is hiring a Reablement Support Worker to help people transition from hospital to home across Warwickshire. You will support with personal care, meals and daily routines, and help clients regain independence following hospital discharge.

Role involves flexible shifts, driving between visits, DBS checks, and full induction with paid training. A compassionate, reliable approach is essential to build confidence and dignity in clients’ own homes.

Qualifications

• Compassionate, patient and reliable with a genuine desire to help others.
• Willing to undertake training and DBS checks as required.
• Flexible and able to travel between visits across Warwickshire.

Responsibilities

• Welcoming people home from hospital and helping them settle in safely.
• Supporting personal care, meals, medication prompts and daily routines.
• Helping people rebuild confidence and practise everyday skills per the reablement plan.
• Spotting changes in health or wellbeing and reporting promptly to the team.
• Keeping clear, accurate records and coordinating with health professionals.
