Job summary

The Care Bureau Ltd is recruiting a Reablement Support Worker to help people transfer from hospital to home and regain independence. You will conduct home visits across Warwickshire, supporting daily activities and medication prompts, while following a personalised reablement plan.

Location: Warwickshire (flexible across the county)

Contract: Guaranteed hours, block shifts

Pay: £12.71 to £12.80 per hour

About the role

Leaving hospital can be daunting. As a Reablement Support Worker with Care Bureau, you'll be the friendly face who gets people safely home and settled again. Like domiciliary care, you'll work in people's own homes. The difference is that you'll support people straight after a hospital discharge. You'll help them settle back in, regain confidence and get back to doing things for themselves, so the aim is to build independence rather than create dependence.

What you'll be doing

• Welcoming people home from hospital and helping them settle in safely
• Supporting personal care, meals, medication prompts and daily routines
• Helping people rebuild their confidence and practise everyday skills, in line with their reablement plan
• Spotting any changes in someone's health or wellbeing and reporting them promptly
• Keeping clear, accurate records and working closely with the wider team and health professionals
