Job summary

Responsibilities

• Build and maintain positive relationships with customers and their family carers, promoting health, wellbeing and independence.
• Support individuals through personalised care plans and short-term enablement strategies.
• Assist with dignified personal care needs and explore alternative ways of meeting long-term needs.
• Promote services that enable safe, independent living at home; monitor progress against care plans and maintain records.
• Contribute to reviews and share information with the team to support ongoing care.
