Job summary

Worcestershire County Council is seeking a Reablement Support Worker in Adult Social Care. You will help people relearn skills and regain independence in their own homes after hospital discharge or crisis, providing hands-on support initially.

Permanent role with day, evening and weekend shifts (7am–10pm) across Worcestershire; requires a full driving licence and access to a vehicle for county-wide travel. DBS checks are required and training is provided.

• Commitment to developing people's lives and a passion for care and support

Responsibilities

• Help people relearn skills and regain independence in their own home after hospital discharge or crisis
• Support personal care such as washing, personal hygiene and dressing
• Support mobility and help with meal preparation
• Enable and encourage service users to do as much for themselves as possible, rather than doing for them
• Give hands‑on assistance initially, reducing as the service user becomes able
• Travel around Worcestershire between clients' homes
