Job summary

Wiltshire Council is seeking a Reablement Support Worker in Chippenham to help people after hospital discharge, working with occupational therapists to co-develop personalised plans. The post is 37 hours per week on a rota basis, with a hybrid working model and a pay rate of £13.92–£14.36 per hour (plus 10% unsocial hours).
