Job summary

Brunelcare is recruiting a Reablement Lead Assistant to work under the Reablement Lead. You will monitor referrals, assess discharge needs, and create reablement plans to support service delivery.

You'll liaise with Health and Social Care professionals, manage equipment requests, handle daily rostering, record client changes, and ensure data is accurately entered. A proactive, flexible team player with IT skills is ideal.

Responsibilities

• Monitor referrals and ensure turnaround deadlines are met.
• Assess needs and goals upon discharge from hospital.
• Create reablement plans to support service delivery.
• Liaise with Health and Social Care professionals to gain the best outcome for our customers.
• Escalate concerns to the Reablement Lead when necessary.
• Oversee and monitor equipment requests.
• Serve as a point of contact for the reablement team, clients and external agencies.
• Complete daily rostering for the reablement visits.
• Record client changes in the system.
• Monitor and input data.
