Job summary

Brunelcare is seeking a Reablement Lead Assistant to support the Reablement Lead. You will monitor referrals, assess needs on discharge, create reablement plans, and liaise with health and social care professionals to deliver outcomes for customers.

The role requires driving with your own vehicle; IT system experience is preferred. You will work closely with the team to ensure deadlines are met and to provide excellent client support.

Qualifications

• Car driving with own vehicle is required.
• IT system experience preferred.
• Proactive, self-motivated and flexible individual.
• Excellent at problem-solving and able to meet deadlines.

Responsibilities

• Monitor referrals and ensure turnaround deadlines are met.
• Assess individuals’ needs and goals upon discharge from hospital.
• Create reablement plans to support service delivery.
• Liaise with Health and Social Care professionals to optimise outcomes.
• Escalate concerns or complaints when necessary.
• Oversee equipment requests and daily rostering for visits.
• Record client changes and input data on the system.
