Job summary

Derbyshire Community Health Services NHS Foundation Trust (DCHS) in Buxton seeks a Registered Nurse to join the Rapid Response team. This Band 5, part-time role (30 hours/week) supports patients at home, preventing hospital admission and aiding safe discharge.

You will provide high-quality community care, assess and plan care, and work closely with GP surgeries and local trusts to maintain patient independence and wellbeing.

Qualifications

• Full NMC registered Nurse
• Evidence of recent relevant training
• Experience of Community Nursing

Responsibilities

• Provide high-quality nursing care to patients in the community to keep them safe at home and support discharge from hospital.
• Assess, plan, implement and evaluate individualized patient care with privacy and dignity.
• Support junior staff and assist sister/charge nurses in managing the healthcare environment.
• Refer to relevant services as appropriate.
• Collaborate with Integrated Community Teams/GP surgeries and local trusts.
• Respond to priority nursing visits.
