Job summary

Derbyshire Community Health Services NHS Foundation Trust in Swadlincote is seeking a part-time Registered Nurse to join the Community Nursing Team on a 12-month secondment. Flexible patterns may be considered to suit the right candidate.

You will assess, plan, implement and evaluate care in patients' homes, support colleagues across practice, therapy and home visiting teams, and travel across a wide area with a vehicle for business purposes.

Qualifications

• Registered Nurse with current registration.
• Ability to travel across a wide geographical area and have access to a suitable vehicle.
• Flexible hours and working patterns may be considered for the right candidate.

Responsibilities

• Assess, plan, implement and evaluate individualised patient care, promoting self-care wherever possible while maintaining privacy, dignity and clinical effectiveness.
• Provide guidance and support to junior staff in delivering high-quality care, while actively assisting the District Nurse/ Senior Community Nurses in managing the healthcare environment.
• Travel across a wide geographical area to support patients in their own homes.
