Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking a qualified nurse to join the North Hardwick Core Community District Nursing Team in Bolsover. This part-time role (30 hours/week) is Band 5 NHS AfC, permanent, with 9-5 core hours Monday to Friday and weekends on a rota.

You will provide high-quality community nursing, assess and plan care, and support junior staff while working across local teams to prevent hospital admissions and support discharge home.

Qualifications

• NMC registered nurse with current registration.
• Evidence of recent relevant training in district nursing.
• Experience of working in the community.
• Understanding of NHS priorities and community health services.

Responsibilities

• Provide high-quality nursing care to patients in the community to support safe home care.
• Assess, plan, implement and evaluate individualised patient care.
• Support district nurses and junior staff; provide guidance.
• Coordinate with GP surgeries and local trusts.
• Travel across Derbyshire to patient homes and attend meetings.
