EAST LONDON NHS FOUNDATION TRUST is seeking an experienced Community Nurse to join the Unplanned Care Night Team as a part-time Band 6 nurse. The role focuses on triage, assessment, and urgent care for housebound patients, with responsibility for cross-cover between North and South Bedfordshire.

You will conduct home visits, manage End of Life Care and catheter care where needed, and liaise with Out-of-Hours GP and palliative services while traveling across Bedfordshire with a driving license.

Job description

Are you an experienced Community Nurse who would like to join our dynamic Unplanned Care Night Team? We are seeking an ambitious, motivated, compassionate and experienced part time Band 6 nurse to support the changing face of community health care. As part of the Rapid Response Team who support the Urgent Community Response, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect at night time.
You will be working as part of the Unplanned Care Night Team to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover between the North and South Bedfordshire areas.
Through face to face patient visits you will carry out nursing care to housebound individuals, often with End of Life Care needs, or for Catheter Management. You will plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.
You will be working closely with responsible for triaging the referrals that come through Single Point of Access between the hours of 8pm until 8am. You will often need to liaise with the Out of Hours GP, and Bedfordshire Palliative Care Services. You will spend your shift working with a Health Care Assistant who you will be responsible for.
This role will include much driving, covering a large geographical area of Bedfordshire. A driving licence and access to a vehicle is required.

Youmusthave a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 20:00-08:00. In either the North or South Areas of Bedfordshire.
