Job summary

Derbyshire Community Health Services NHS Foundation Trust (DCHS) in Buxton seeks a Registered Nurse to join the Rapid Response team. This Band 5, part-time role (30 hours/week) supports patients at home, preventing hospital admission and aiding safe discharge.

You will provide high-quality community care, assess and plan care, and work closely with GP surgeries and local trusts to maintain patient independence and wellbeing.

We are looking to recruit a NMC registered nurse to work within the Rapid Response team in High peak and Derbyshire Dales Rapid response team

Main duties of the job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
